MADISONVILLE SANDWICH SHOP RECEIVES CRITICALLY LOW HEALTH SCORE OF 45

On September 10, 2025, Henry’s Burgers and Subs at 4914 New Highway 68 in Madisonville, Tennessee, received a failing health inspection score of 45 out of 100 following a complaint-driven inspection.

The restaurant faced numerous critical violations, including live insects crawling in food preparation areas, a reach-in cooler with food, and around dish storage, prompting a requirement for a certified pest control company visit within 10 days.

The grill was covered in excessive grime, a crockpot contained old chili, and spoiled celery and outdated salsa (dated mid-July) were discarded.

The person in charge demonstrated a lack of managerial control, unable to answer basic food safety questions, and utensils were not cleaned or replaced every four hours as required.

Additional issues included improper food storage, lack of hand soap in the women’s restroom, melted and damaged equipment, and overall poor sanitation with grime buildup on surfaces and floors.

A follow-up inspection is pending to address these violations, and warning letters will be requested for repeat issues like improper date marking and failure to post the latest inspection report.
