Immunizations and physical examinations may be given by authorized personnel at Rhea County Health Dept. or by parents’ private physicians. Please take your present immunization form to registration. Please check with your healthcare provider to be sure that your child meets these new requirements.
4.Social Security Cards(optional). These are not required but would be extremely helpful for computerized attendance and testing records. Applications may be obtained by calling 1-800-772-1213 and requesting that an application be mailed to the home address of parents/guardians.
5. The stub of an April or May utility bill—such as electric, cable, or gas, showing your correct residence address, not a P.O. Box. This will be proof as to which school your child should attend.
If all required information is not provided at registration, the child’s name will be placed on a waiting list and will then be taken on a first-come, first-served basis when requirements are met. Each school will send its overflow to the nearest school that has an opening.

Anyone who has questions may call the Rhea County Department of Education at 775-7812 or the school where the child will register. Thank you for your cooperation and assistance.
